Blue Choice PPO
Your BlueChoice® for Individuals Under 65 PPO insurance plan includes predictable copayments when you select a participating PPO Family Physician to provide your care. With BlueChoice® you also have the option, at point of service, to receive care from any physician, provider or hospital participating in our Traditional provider network, where you receive balance-billing protection. A child-only policy is available for dependent children ages 1-17.
